This Waiver Agreement is a legal agreement between you and One Two Dream, Inc. DBA "Dream League".

• Players who have not submitted an online waiver will not be allowed to step foot on the court.
• If you change your name or address or phone number, you must submit a new online waiver.
• All required fields must be entered for this form to be valid.
• Your name, address and phone number are required for legal purposes.
• Your information is considered private and confidential and will not be sold to third parties.
• If you are under eighteen (18) years old, contact us at info@dreamleague to obtain a parental permission form.
• There are four (4) parts to this agreement and the fourth and last part is optional.

PART I -- Waiver of Liability, Trademark, and Management Agreement

I hereby acknowledge that I am over the age of eighteen (18) years as of the date of submission of this form and have read, understood, and abided by the information on this Waiver of Liability, Trademark, and Management Agreement. There are risks associated with participation in Dream League's basketball leagues, tournaments, and related activities. On behalf of myself, my heirs, executors, administrators and assigns, I hereby waive, release and discharge Dream League, host facilities, business partners, event sponsors, event charities and any of the agents, employees, officers, and directors of the foregoing from any and all claims for damages, actions or suits and demands whatsoever in law and in equity, including but not limited to, the risk of injury from playing in and traveling to and from this basketball league or tournament, arising out of any ordinary negligence, first aid rendered, treatment, drugs, medicines, or surgical procedures performed in case of emergency, and risk of loss of personal property by theft or otherwise. I realize that basketball is a very physical game and that I am playing at my own risk. I will inspect the facility and surrounding areas for safe playing and parking conditions. I will be responsible for providing my own medical insurance and/or first aid equipment should an injury occur. Furthermore, I hereby grant full permission for event organizers to record any or all of my participation in this event for photos, motion pictures, TV, radio, recordings, videotapes and other media known or unknown, and to use them no matter by whom taken, in any manner for publicity, promotions, advertising, trade or commercial purposes, without any remuneration of any kind due me.

PART II -- Oath of Sportsmanship, Conduct, and Community Service

1. I have read and understood the rules and fines pertaining to the Zero Tolerance Rule.

2. I understand that the chain of communication must be made through the team captain.

3. I understand the scheduling parameters, forfeit, and pro-rated time rules.

4. I understand the jersey rule and the substitution rules.

5. I pledge to increase the awareness of the needs of underprivileged youth from disadvantaged neighborhoods through my participation in Dream League.

6. I understand that a portion of any contributions to Dream League may be tax deductible and I will consult my tax advisor for further details.
